medical history for oral surgeon–transfer to wdc

can we install the medical history for Avenue Dental at Westgate Dental so that when pt's comeplete the medical history online for the oral surgeon it now comes to WDC instead of Avenue as we will no longer be working there as of now! We will be starting our first day at wdc next week! THanks! If this can be done asap it would be greatly appreciated!
